Supporting the Transition From School to Adult Life With Purpose and Skill

The Transitional Vocational Services Program (TVSP) at CREATE Independence is designed for adolescents and young adults who are navigating the shift from high school to adulthood. This individualized program supports participants in building essential life skills, increasing social and community engagement, and preparing for meaningful vocational experiences.

With a focus on person-centered planning, daily routines, and real-world learning, TVSP helps individuals develop the confidence, self-advocacy, and functional abilities they need to participate in life with greater independence.

The Transitional Vocational Services Program helps participants build the foundation for adult life. These focus areas reflect the skills and experiences that prepare young adults for the next step in education, employment, or independent living.

On-the-Job Training

Individuals gain real work experience in community settings while developing practical job skills with support from staff and coaches.

Pre-Vocational Training

Exposure to job readiness concepts such as workplace behavior, task completion, resume preparation, and participation in volunteer experiences.

Daily Living Skills

Hands-on development of self-care, meal preparation, home routines, transportation safety, and wellness planning.

Social and Community Participation

Opportunities to build relationships, practice communication, and engage in structured community outings and activities.

Goal Setting and Self-Advocacy

Guidance in identifying personal goals, advocating for support needs, and building confidence in decision-making.

Functional Education

Instruction in life skills that support independence in home, school, and community settings, including money management, time management, and communication.

Day Services Option (DSO)
